The Transition House
The Outpatient Center by The Transition House offers individual, group and family counseling you need to restore your health and self esteem. Services are available for Children (seven years and older), Adolescents, Adults, and Older Adults.
We offer help for a wide range of emotional and behavioral issues and provide services that span from therapy for depression, substance abuse, to grief counseling and beyond. Our Outpatient Center provides a comfortable and supportive atmosphere where we offer a highly personalized approach tailored to each of our clients’ individual needs to help attain the personal growth they’re striving for.
Our Homeless Veterans Residential program offers services to male and female homeless veterans, chronically mentally ill homeless veterans, veterans with PTSD diagnosis, homeless veterans and their families, homeless veterans with substance abuse problems, homeless veterans with dual diagnosis, veterans being released from prison, disabled homeless veterans and homeless veterans with mental illness.
The Transition House Inc. operates a very structure program that is client-centered and client-friendly, accomplished in a setting that makes our veterans feel a part of their community.
Mens Residential Program
Our Mens Residential program provides services for those dealing with behavioral health issues such as substance abuse, mental health and other co-occurring disorders. Our services include: Short term/long term residential program; Case Management; Individual Counseling; Group counseling; Psycho-educational groups; Individualized treatment planning; Employment skills group; assistance with job placement; and 12 Step Recovery groups.
Each individual participates in their treatment planning, to achieve the most out of the program while they are here. Our staff works to guide our clients to build a foundation while in the program to utilize when they move on. Utilizing a strength based, empowerment model is key to guiding individuals to be successful upon completion of their program.
Victory Village is our Permanent Supportive Housing program, designed to provide permanent housing and supportive services that will allow homeless families and individuals the ability to live independently.
It provides help that give homeless families and/or individuals the ability to achieve residential stability, increase their skill levels and/or incomes, and obtain greater self-determination and self-esteem.
We are a contracted provider for the Department of Corrections, State of Florida. We offer a two-part program based on a recovery treatment approach to substance use and criminal behavior that stresses individual and group performance, personal and community accountability and cognitive and behavioral change.
Our two-part program in Kissimmee, FL is conducted as a modified Therapeutic Community. The Therapeutic Community represents a highly structured environment with defined boundaries, both moral and ethical. It employs community-imposed sanctions and penalties, as well as earned advancement of status and privileges as part of the recovery and growth process.Check it out
